In the case of over-extended nipples, a ring-shaped (annular) piece of nipple skin is removed at the base of the nipples without disturbing the milk ducts. The first incision in the skin of the nipples is made in a circular fashion at the junction with the areola, and the second is made a few millimeters above on the skin of the nipple itself.
